Your Rights and Responsibilities are provided to you for review on your first visit to UMC and annually thereafter. In addition, this is posted at all clinic sites.


Privacy:
UMC is committed to providing you with the privacy in which to conduct your medical and financial business. Even though some facilities have physical constraints, we will do our best to accommodate your needs.


 

Informed Consent:
You have the right to decide what will be done to your body during the course of medical treatment.

 

 

Advance Directives:
You have the right to determine in advance what health care decisions will be made and who will make them if you are not able to make them yourself

 

Continuity of Care:
UMC offers services to complement your follow-up care. These include Laboratory, Radiology, Pharmacy, Dental, Podiatry, and referrals to Medical Specialists.

A broad array of primary and specialty care is provided to our patients by a diversity of health care providers and staff, many of whom speak both English and Spanish. Most of the physicians working at UMC also have hospital privileges and can make arrangements for patients to be admitted to our local hospitals.

The Dental clinic provides examinations, cleanings, fillings, and extractions.

Women, Infants, and Children Nutrition Program (WIC)
This program provides nutrition education and food vouchers to low-income mothers who are pregnant, postpartum, or breast feeding, and to children under age 5.

 

Medication Assistance

Low-income patients may qualify for discounted prescription medications through our Pharmacy Assistance Program and Medicare Pharmacy Assistance Program.
