If you're concerned with payment, please speak to one of our bilingual Financial Counselors. We are committed to guiding you through our discounted Federal sliding fee schedule and assist you through the eligibility process.
To see if you are eligible for the discounted sliding fee schedule please stop by one of our offices and bring the following required information.
1. Most recent Income Tax Form. If you are self employed bring your 1040 & W2 form or two most recent check stubs, include any unemployment benefits, child support, Temporary Assistance For Needy Families TANF, or any other type of income.
2. Proof of Government Income, AFDC Reports, Pension Reports, Annual Report of Social Security Benefits or Supplemental Benefits.
3. Picture ID's
4. Most recent Utility Receipts, e.g., Electricity, Water, Gas or Rent receipts. *Note: Your physical address is required to be listed on all the Utility Receipts mentioned above.
